A mental health evaluation is a structured assessment done by a trained professional (like a psychiatrist, psychologist, or therapist) to understand your emotional, psychological, and behavioral health.
What it’s for
- Identify mental health conditions
- Understand symptoms (mood, thoughts, behavior)
- Guide treatment (therapy, medication, or both)
- Provide documentation (for work, school, court, or immigration in some cases)
What happens during the evaluation
It’s mostly a conversation, not a physical exam. You may be asked about:
- Your current symptoms (anxiety, sadness, stress, sleep issues)
- Medical and mental health history
- Medications or substance use
- Family history
- Daily functioning (work, relationships, routines)
The provider may also assess conditions like:
- Depression
- Anxiety disorder
- Post-traumatic stress disorder

- Basic screening: short questionnaires PHQ 9, GAD7 and Suicidal risk forms
- Comprehensive evaluation: detailed interview + possible tests
What you get after
- A summary or diagnosis (if applicable)
